Job Description

Occupational Therapist – Home Health

Part-Time | Flexible Scheduling | Southeast Iowa

Looking for an Occupational Therapist role where you can have more autonomy over your day, provide meaningful one-on-one care, and be part of a team that puts quality ahead of cutting corners?

Advanced Home Health Care is hiring a part-time Occupational Therapist to provide in-home therapy services throughout our Southeast Iowa coverage area, including Des Moines, Lee, Henry, Louisa, and Van Buren Counties.

We offer a  competitive visit rate , flexible scheduling, mileage reimbursement, and the opportunity to play an important role in our growing internal therapy program.

Why Join Advanced Home Health Care?

We're building our own internal therapy team so our Occupational Therapists can be an integrated part of one coordinated care team, rather than a separate contracted service. Our goal is simple: provide patients with consistent, high-quality care from professionals who truly feel like part of the Advanced Home Health Care team.

As a Home Health Occupational Therapist, you'll also have greater  flexibility and autonomy over your workday . Rather than spending each day in the same clinical setting with back-to-back appointments, you'll work throughout the community, build meaningful relationships with patients and families, and organize your patient visits around their needs and your schedule.

What we offer:

  • Competitive hourly compensation
  • Flexible scheduling based on caseload and the autonomy to manage patient visits throughout our Southeast Iowa service area
  • Provider-friendly charting software
  • Mileage reimbursement for community travel
  • Supportive, interdisciplinary team environment

What You'll Do

  • Conduct occupational therapy evaluations and develop individualized treatment plans
  • Help patients improve their ability to perform activities of daily living (ADLs), including dressing, bathing, grooming, and other daily routines
  • Assess home environments and recommend modifications or adaptive equipment to improve safety and independence
  • Provide therapeutic activities to improve fine motor coordination, upper extremity function, and functional mobility
  • Educate patients and caregivers on adaptive techniques, energy conservation, and home safety
  • Monitor patient progress and collaborate with the interdisciplinary care team to support patient goals
  • Supervise our COTA staff to ensure treatment plans are followed

What We're Looking For

Experience Requirements:

  • Active Occupational Therapist license
  • Experience in outpatient, inpatient, rehabilitation, or other clinical settings welcome
  • Previous home health experience preferred, but not required
  • Strong communication and organizational skills
  • Collaborative, patient-centered approach
  • Commitment to providing high-quality care and improving patient outcomes
  • Ability to independently manage patient visits, treatment plans and documentation in a home health setting
